Plexr INFORMATION AND CONSENT FORM

Information

Swelling, scabs and redness are part of normal skin renewal. These effects last on average from 7 to 20 days depending on the treatment performed

It is recommended:

• To wash the part with neutral Marseille solid soap, both in the morning and in the evening

• To avoid exposure sunlight and cover the part with 50+ SPE foundation until the redness disappears to avoid hyperchromia (darker spots),

• Do not remove the crusts for any reason, which must fall independently in order to avoid depressions

and irregularities of the skin.

In darker-skin subjects such discolorations may occur more likely

In those fair-skinned and with sensitive skin, the redness may persist longer. In both cases ask the doctor how to accelerate the restoration of the normal color, which will return to normal even without

treatments after a maximum of 4 months

Aftercare Kit

A set of 3 essential skincare has been designed by Uplifting Skincare Lab for Plexr. We highly recommend the use of the Plexr Care Kit to optomize the results and reduce recovery time. This kit includes:

Uplifting Healing Concealer

A special aftercare camouflage concealer has been designed for Plexr to cover the seabs, protect the skin and reduce recovery time. Start using it right after the treatment and repeat every 3 to 5 hours until the skin back to its normal condition

• Uplifting Eye Care. Vit K & Dermaxyl Vitamin K is a little-known compound that excels in both hydration and healing and is especially useful in the fight against skin conditions such as hyperchromia of the treated area

Dermaxyl is an anti-aging, wrinkle smoothing and cutaneous barrier repair ingredient. It stimulates cell communication and then repairs the skin damage. If the patient’s home care protocol is not strictly followed, it may result in important dyschromia,

depressions, infections and possible inflammation. In this case, consult your doctor immediately

Exosomes Post Care

POST CARE FOR INJECTED EXOSOMES FOR PAIN RELIEF

● What to Expect After Treatment: ​Immediately following the procedure, the most commonly reported temporary side effects are redness, swelling, bruising, tenderness, tingling, numbness, lumpiness, and/or a feeling of pressure or fullness at the injection sites and/or in the treated area(s). Cold gel packs/ice may be gently applied immediately after treatment to reduce swelling. The procedure may initially increase the symptoms for one to three days (and occasionally, up to ten days), and then may decrease in intensity.
● To Avoid Bruising​: Avoid alcohol consumption for a minimum of 6 hours and refrain from taking blood thinners such as Aspirin and NSAIDS for several days. Tylenol is recommended if needed for discomfort.
● To Maximize Results and Prevent Complications: ​o Avoid touching or scrubbing at the injection sites for 24 hours after treatment. o Avoid direct high heat (blow dryer, sun exposure, sauna, steam room, very hot shower, hot yoga, strenuous exercise, etc.) for 24 hours after treatment.
● Maintenance Treatments: ​The results of this treatment can last up to more than 1 year, but results vary and research documenting the longevity of results is ongoing. Follow up visits are recommended every 6-12 months. POST CARE FOR TOPICAL APPLICATION OF EXOSOMES WITH MICRONEEDLING
● To Maximize Results: ​For a minimum of 24 hours after the Exosomes has been applied to your skin, do NOT: Wash your skin, expose the treated area to direct high heat, or engage in activities that will get the skin wet or cause you to sweat (blow dryer, sun exposure, sauna, steam room, Jacuzzi, very hot shower, hot yoga, strenuous exercise, etc.). It is recommended to not apply makeup for a minimum of 24 hours after treatment.
● For best results and efficacy: ​We recommend follow up visits at 6-12 month intervals. You may notice immediate as well as longer term improvements in your skin.
● Combination Therapy for Optimal Results: ​Exosomes Therapy stimulates your skin to grow new, younger tissue however; it does not prevent muscle movement or resurface the skin. Most patients see best results when combining their PRP treatments with other anti-aging procedures such as Botox®, HA Filler, AFT, FOCUS, etc.

POST CARE FOR TOPICAL APPLICATION OF EXOSOMES FOR HAIR TREATMENT FOR HAIR LOSS

It is not necessary to make many changes in your plans today. Do whatever you wish with the exception of avoiding aspirin advil, and alcohol. It is quite normal to experience some mild soreness and rarely swelling, Not all individuals experience this, but it can occur. Let us know if you have concerns and need our help.

● Shampooing: ​Please shampoo your hair tomorrow morning with any shampoo you wish
● Hair Coloring:​ It is fine to color your hair three days after the procedure
● Hats:​ Wearing a hat today is fine if you wish to. In fact, please limit sun on the area for 2 days.
● Supplements:​ Do not use blood thinning agents such as vitamin E, vitamin A, Ginko, Garlic, Flax, Cod Liver Oil, Essential Fatty Acids and least one week after your treatment
● Foods and Caffeine:​ Avoid alcohol, cigarettes for the next three days.
● Smoking:​ Avoid smoking for three days (if possible) – we find it makes a big difference to healing.
● Medications​: Avoid aspirin, Advil, Motrin, Iburprofen, Naproxen, Voltaren and other anti inflammatory medications for 3 days after your procedure (if possible try for 7 days). We want inflammation to occur!

POST CARE FOR EXOSOMES HAIR RESTORATION INJECTION TREATMENT

Please carefully read and follow these instructions after your Exosomes treatment. There are minimal restrictions after your Exosomes injections allowing you to return to your daily activities almost immediately.

● DO NOT touch, press, rub or manipulate the treated area(s) for at least 12 hours after your treatment.
● AVOID Advil, Aleve, Aspirin, Ibuprofen, Motrin, Naprosyn, (all non-steroidal anti-inflammatory agents), Vitamin A, Vitamin E, Gingko Biloba, Garlic, Flax Oil, Cod Liver, Essential Fatty Acids (EPA, DHA), for at least 1 week prior to and 2 weeks after your treatment. Remember, our goal is to create inflammation. These listed medications will limit or prevent inflammation, which can diminish your results. If you must take Aspirin for cardiac reasons, you certainly may do so, but this may limit your results.
● It is normal to experience bruising, redness, itching, swelling and/or soreness that may last from 2-5 days following your procedure. If you experience any pain or discomfort you may take Tylenol or other Acetaminophen-containing products as directed.
● We would prefer if you could refrain from applying ice to the injected area as ice acts as an anti- inflammatory.
● Do not wet your hair for at least 3 hours after your treatment.
● For the first 3 days, use shampoo that is pH balanced.
● Do not use any hair products for at least 6 hours after your treatment.
● AVOID saunas, steam rooms, swimming for 2 days after your treatment.
● AVOID vigorous exercise, sun and heat exposure for at least 2 days after your treatment.
● AVOID alcohol, caffeine, and cigarettes for 3 days after treatment. Smokers do not heal well and problems recur earlier and results may take longer.
● AVOID resuming Minoxidil (Formula 82M, Rogaine®) or hair coloring or straightening for 3 days.
● Continue increased water intake the first week after your treatment. Please do not hesitate to call our office should you have any questions or concerns regarding your Exosomes treatment or aftercare.

Thread lift aftercare instructions

1. Apply SPF30 + and avoid direct sunlight & tanning machines in order to prevent post Inflammatory Hyperpigmentation (PIH).

2. Gently wash the treated areas on the same day but do not rub or massage the face for 2 weeks.

3. Try to sleep with either two pillows on your side (or by using a travel pillow) in order to avoid pressure at night.

4. If you received threading in other areas rather than face, avoid to press those areas during day or night time.

5. In case of genital threading, avoid sexual activity for 2-3 weeks following the procedure.

6. Do not open mouth too wide for 3-4 weeks (avoid dental treatment for 2 weeks if possible)

7. A stinging or “pulling” pain or discomfort is normal. Take Tylenol 500mg 1-2 tablets every 6 hours as directed.

8. Avoid taking any anti-inflammatory medication such as Naproxen or Ibuprofen, because inflammation is necessary to initiate the new collagen formation process. Ibuprofen and similar medications will suppress this process.

9. Avoid alcohol and anti-coagulant medicines (aspirin, unless prescribed for medical indications) for 7 days.

10. Avoid blood ‘thinning’ vitamins C, A & E for 5 days.

11. Taking Vitamin C 2000 mg/day for 2 weeks start from 5 days after thread lift implant.

12. Avoid temperature extremes such as sauna or very cold climate for 15 days.

13. Avoid physical exercise for 15 days post procedure.

14. Avoid having Radio Frequency (RF) or any heat producing device (laser, IPL, etc.) treatments of the thread treated areas for at least 8 weeks.

15. Anti-inflammatory medication may be taken after 7 days.

16. Patient must finish course of antibiotics if prescribed as part of the treatment protocol.

17. Stinging or “Pulling” pain discomfort is normal, this will subside over a 2-3 week period.

18. Scratchy sensation is normal; this will subside over a 2-3 week period.

19. Stinging, “Pulling”, scratchy sensations will all disappear by week 5.

20. Possible complications may include Infection, swelling, bruising, foreign body reaction in epidermis (granuloma) and protrusion.

21. Aware your plastic surgeon If you want to apply for face lift, thread lifting might affect the procedure by possible fibrosis in implanted area in some cases.

22. Extremely rare, temporary facial nerve fall out may occur due to local anesthetic, swelling, hematoma or pressure of the cannula or thread on the nerve. Your practitioner MUST be informed immediately if facial nerve fall out is experienced and if your practitioner can’t be reached then go immediately to the Emergency Department.

23. Follow up examination 7 days later.

24. Combination with other procedures from 7 days onwards: PRP, MicroPen, fillers.

25. Follow up photography at 1-month post treatment. Note: treatment of untreated areas may be initiated at any time.

26. End-point follow-up appointment at 3 months post procedure.

*If your practitioner canʼt be reached, go immediately to the Emergency Department.

Platelet Rich Plasma (PRP) Therapy Pre and Post Care

PRECARE INSTRUCTIONS: TO PREPARE FOR THE BLOOD DRAW ASSOCIATED WITH PRP THERAPY

● PRP Therapy is very safe because cells from the patient’s own blood are used, which means there are no preservatives and no chance of the body rejecting the cells. The primary risks and discomforts are related to the blood draw where there is a slight pinch to insert the needle for collection and there is a potential for bruising at the site.
● For optimal results and to decrease the chance of bruising at the draw site, please avoid all blood thinning medications and herbal supplements for 1 week prior to your appointment. Avoid taking Aspirin and non-steroidal anti-inflammatory medications (NSAIDS) such as such as Ibuprofen, Motrin and Aleve. In addition, very high doses of some Vitamins and supplements can thin your blood and increase the chance of bruising. Please notify your provider if you are taking Coumadin, Plavix, or any other blood thinners for a medical condition.
● During the course of your treatments, notify our staff of any changes to your medical history, health status, or personal activities that may be relevant to your treatment.

POST CARE FOR INJECTED PRP WITH VAMPIRE FACELIFT®

● What to Expect After Treatment: ​Immediately following the procedure, the most commonly reported temporary side effects are redness, swelling, bruising, tenderness, tingling, numbness, lumpiness, and/or a feeling of pressure or fullness at the injection sites and/or in the treated area(s). Cold gel packs/ice may be gently applied immediately after treatment to reduce swelling.

● No Facials, Facial Massages, or Laser Treatments for 2 Weeks Afterward: ​Most facials, chemical peels, laser, and light treatments may be done immediately ​prior ​to the treatment but not for 2 weeks after the treatment. Botox® may be injected immediately before or after.
● Combination Therapy for Optimal Results: ​PRP Therapy stimulates your skin to grow new, younger tissue however; it does not prevent muscle movement or resurface the skin. You ​must ​wait ​2 ​weeks ​before retreating ​or ​correction.

****Please ​report any redness, blisters, or itching immediately if it occurs after treatment.****

● You may experience slight redness and swelling, which should resolve within 24 hours.

● Mineral make-up can be worn immediately following treatment.

● Avoid heat, saunas, hot tubs and sweaty activity for 24 hours; this includes exercise of any kind.

● Avoid products containing any exfoliating agents (retinoic acid, retinol, tretinoin, retinol, benzoyl peroxide, glycolic acid, salicylic acid, astringents, etc.) for 24 hours after treatment.

● You may resume your regular skin care routine 24 hours after treatment.

POSSIBLE RISKS OF GLUTATHIONE:

● Possible allergic response to IV infusion of glutathione.
● Theoretical interaction with Chemotherapy and/or Radiation therapy thereby decreasing
the effectiveness of those treatments.

CONTRAINDICATIONS:

POSSIBLE SIDE EFFECTS:

● Herxheimer reactions: body aches, nausea, occasional headaches, mild diarrhea and chills without fever.

Keep in mind that some of these (Herxheimer reactions: body aches, nausea, occasional headaches, mild diarrhea and chills without fever), fatigue skin eruptions flu-like symptoms, can be due to detoxification effects stimulated by Glutathione.

A few simple guidelines both pre and post-treatment can make a difference between a good result and a fantastic one.

1. Do NOT, touch, press, rub, massage, or manipulate the treatment area.
2. Ice the treated areas for the next 24 hours. Place the icepack on the area for 20
minutes and remove the ice pack for 20 minutes. Continue this pattern for 24 hours.
3. Apply (over the counter) Arnica gel to the treatment area to help decrease the bruising,
swelling, and discomfort.
4. Take (over the counter) acetaminophen and/or pain reliever if needed to decrease
post-treatment discomfort.
5. Sleep on your back and with your head elevated for the next 3-5 days after treatment.
6. Drink plenty of water and fluids after treatment.
7. Avoid vigorous exercise, sun and heat exposure for 3-5 days after treatment.
8. Avoid Aspirin, Motrin, Gingko Biloba, Garlic, Flax Oil, Cod Liver Oil, Vitamin A, Vitamin
E or any other essential fatty acids at least 3 days to 1 week after treatment. These
items may increase bleeding and bruising.
9. Avoid alcohol, caffeine, Niacin supplement, high-sodium foods, high sugar foods,
refined carbohydrates, spicy foods, and cigarettes 24-48 hours after your treatment.
These items may contribute to increased swelling or irritation.
10. Avoid cosmetic treatments such as injectables, laser, ultrasound, peels, facials or
micro-dermabrasion to the Kybella treatment area. Ideal Image can assist and advise in
coordinating specific treatment timing with other concurrent aesthetic services.
11. Please report to your provider immediately if you develop an asymmetric smile or facial
muscle weakness, skin ulceration in the treatment area, difficulty swallowing, or if any existing symptom worsens.

Neuromodulators (e.g. Botox, Xeomin and Dysport) are meant to temporarily block the transmission of nerve impulses to the muscles – reducing the activity in these muscles which decreases the appearance of lines/wrinkles.

After placement into the targeted muscle, the weakening effect gradually begins anywhere from 24 hours – 5 days and is not complete for two weeks. Therefore, optimal results are not seen for at least two weeks. Any assessment for touch-ups cannot be performed until 2 weeks after the initial treatment.

Instructions:

● Do not rub the site.
● You may experience stinging at the injection site for 10-15 minutes
● You may experience temporary redness and/or bumps. This usually subsides in
a few hours.
FOR 24 hours unless otherwise noted (to allow product to settle evenly):
● Do not bend over.
● Maintain an upright posture for 6 hours.
● No vigorous exercise for 12 hours.
● Do not lie down flat for 4 hours. If you lie down, lay at an incline.
● Do not lie on your side. Lay on your back at an incline.
● Do not consume alcohol (thins blood)
● Do not take Vitamin A
● APPLYING MAKEUP – For 24 hours after treatment please wipe the treated areas with alcohol before and after you apply makeup to protect from bacteria
● Do NOT take Aspirin. Some patients develop a headache, which can be treated with Ibuprofen or Tylenol. This usually lasts less than 24 hours.
● For 24 hours, do not have any facial treatments, micro-dermabrasion, or electrolysis
● Avoid direct pressure to injected areas (such as brow) with visors, hats, cleansing brushes, or massage tables for one week

Vitamin C Injections side effects include but are not limited to:

• Mild soreness
• Temporary dizziness
• A feeling of pain and headaches
• With rare instances of bowel intolerance or diarrhea.

If any of these side effects become severe or troublesome I will contact my physician immediately.

Vitamin B12 is safe and non-toxic even when taken in high doses, however I understand that it is possible that I could have an adverse reaction, though rare they can include: mild diarrhea, anxiety/panic attacks, heart palpitations, insomnia, breathing problems, chest pain, skin rashes/hives.

Most common side effects are redness/swelling and soreness around the injection site lasting up to a few days.

We strongly recommend all of our clients to form a relationship with a Primary Care Provider and have regular check-ups. Pain/redness at the injection site, mild diarrhea, itching, or a feeling of swelling all over the body may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.rare but very serious symptoms occur: chest pain (especially with shortness of breath), weakness on one side of the body, sudden vision changes, slurred speech.

Get medical help right away if you have any very serious side effects, including symptoms of heart failure (such as shortness of breath, swelling ankles/feet, unusual tiredness, unusual/sudden weight gain).
